Discussion Topic:  Ethical use of AI in Clinical Decision Support Systems

 In the recent issue of the BMJ December 2021 – Volume 47 – 12, there is a paper entitled “Primer on an ethics of AI-based decision support systems in the clinic” (https://jme.bmj.com/content/47/12/e3 ), that looks at the changing environment with the adoption of AI as a decision support mechanism.

I am interested to know thoughts on this as in my opinion there are a number of advantages that outweigh the disadvantages; but the ethical dilemma exists as to whether a machine programmed to understand and determine an outcome has the ability to make a confirmed decision or whether that should remain with the human, ie Health Care Practitioner, in all cases.

AI has a part to play and where large volumes of data are available to support an AI decision where information overload could make the decision a fuzzy process, as alluded to by El-Kareh and Sitting in their article “Enhancing Diagnosis Through Technology: Decision Support, Artificial Intelligence, and Beyond.”  (Critical care clinics [Crit Care Clin] 2022 Jan; Vol. 38 (1), pp. 129-139.) where they suggest AI will reduce the burden of data but still see it as vital that a CDS supported by AI, or not, requires the cognitive processes of a frontline clinician.

So if we narrow the question down to a topical scenario where AI could be, and is being used, to review cancer images what do you feel about:

As the cancer images are able to be compared at pace to millions of similar known images and their clinical outcome using a developed AI algorithm which has the potential to be 99.9% correct in spotting a known tumour; can the clinician rely on that decision, or outcome?
